You are invited to participate in community health needs survey conducted by Boise State University

Saltzer Health and Intermountain Healthcare are joining other local health systems, public health departments, and community partners working together to create a Community Health Needs Assessment (CHNA) for publication in June of 2023.
The 2023 CHNA will provide information that will help these organizations better understand gaps in services and align resources to meet the health needs of our community.
The study covers Ada, Adams, Baker, Blaine, Boise, Canyon, Elmore, Gem, Jerome, Malheur, Owyhee, Payette, Twin Falls, Valley and Washington counties.
Residents of the 15-county area of Southern Idaho and Eastern Oregon are invited to participate in a survey conducted by the Idaho Policy Institute at Boise State University.

The survey, which takes 10-15 minutes to complete, is anonymous and can be translated to Spanish, Kiswahili, Somali, Russian.
